Thoughts after last night's shift

I predict that fifteen years from now, in an eight hour shift an ER doc will spend about an hour actually diagnosing and treating two patients, and seven hours on the documentation necessary to satisfy the payors, JCAHO, EMTALA, other state and federal regulators, and the plaintiff bar with respect to the care of those two patients.

The average wait for a patient to be seen by a physician and treated in an ER will be 36 to 48 hours.

Wealthy patients near the country's borders will go to Canada or Mexico and pay cash. Poor patients will be served by a system of self-trained 'lay doctors'.

Newspapers will publish outraged editorials about the situation. Politicians will pass laws. These laws will necessitate more even documentation, so that ER docs will only be able to see a single patient in an eight hour shift.

I will be retired. I will laugh. Ha-ha! Suckers!
